• The YMCA of Roanoke Valley is a chartable non-profit organization that provides wellness in our community from the generosity of our donors. • YMCA Membership ID cards must be presented to enter the facility. Replacement ID cards may cost up to $5.00 each. • Loaning your membership card to another person will be subject to loss of membership privileges. • Members are encourages to secure personal items in a locked locker. • The YMCA will not be liable or responsible for personal injuries, loss, or damage to personal property of the member during use of the facilities. • The YMCA reserves the right to suspend or terminate a membership if the member’s behavior is not consistent with the YMCA’s Code of Conduct. • Membership dues are collected by automatic draft on a monthly basis and are continuous unless written cancellation notification is received. • Membership dues paid annually and joining fees are non-refundable. • The YMCA of Roanoke Valley requires a 30-day written notice for cancellation of membership. • The YMCA of Roanoke Valley requires a 14-day written notice prior to the draft date for changes of bank account or card number changes. • Returned Drafts or Returned Checks are subject to a $25 service charge or return fee. • The YMCA Volunteer Board of Directors may adjust the membership rates by giving the member a 30-day written notice. • I authorize the YMCA to photograph any person listed on this form to use for promotion of YMCA activities
